What is Trinity MOMS
Trinity MOMS stands for Maternal Outcomes Monitoring and Support. We are a support program for eligible pregnant and postpartum patients who may need extra help staying connected to care.
We know that when you live in a rural or hard-to-reach area, getting the support you need isn't always easy. Appointments may be far away. Transportation may be difficult. Questions may arise when help does not seem close by.
Trinity MOMS helps bridge those gaps by connecting mothers to telehealth support, education, helpful tools, and community resources so they can feel more supported throughout pregnancy and after birth.
